It may not have been an innovation but rather a restoration. If you read Harry Carr's account carefully you might even discover who reintroduced it

In China with the Tiandhui a "beneficial society" with a degree system and ritual, secret words signs and grips. In Japan with the "heaven amd eath" in Mexico with Zapata and so forth. If you read Carr's material while wondering about private (bloodline) rituals (as in Holy Blood Holy Grail) then you may be able to identify who may well have introduced the 3rd degree, when and where.
Why it was introduced is another question that is much more interesting - for me at least.
